Member Guidelines

1. What Gabriel Care Is

  • Wellness coverage membership for eligible proactive care
  • Separate from ACA Marketplace plans and other compliant insurance
  • Insurance plans are issued and billed by licensed carriers
  • Gabriel provides care navigation, reimbursement support, and health intelligence

2. Membership

  • Open to US residents 18+
  • No religious requirements
  • Monthly membership based on selected tier
  • Membership begins on payment date
  • Cancel anytime with 30 days notice

3. Wellness Benefit

  • Starting wellness balance depends on selected tier
  • Wearable-linked benefits may increase monthly value when available
  • Resets monthly on billing date
  • Does not roll over
  • Eligible categories may include holistic doctor visits, peptide protocols, advanced labs, IV therapy, supplements, cannabis medicine where available, psychedelic therapy where legal, massage therapy, acupuncture, and related proactive care
  • HSA/FSA eligibility depends on the underlying expense and IRS rules. Consult your tax advisor for specific eligibility questions.

4. How Claims Work

  • Submit receipt via Gabriel chat or app
  • AI verification checks: eligible service, fair market pricing, within balance
  • Approved claims are typically reimbursed within 24-48 hours depending on tier and documentation
  • Reimbursement via linked bank account (Stripe Connect)
  • Gabriel Preferred Providers (in-network): 100% reimbursement up to your wellness credit balance
  • Any other provider (out-of-network): 80% reimbursement. You can see any licensed provider you choose.
  • Fair market price: Gabriel may adjust reimbursement to fair market rate if submitted amount exceeds typical pricing
  • Claims above $500 require chat-based pre-approval before treatment (approval is typically instant via Gabriel AI)
  • Benefit unlock: Some Gabriel benefits may unlock over time as your health profile, documentation, and personalized protocol are completed.

5. What's Not Covered

  • Recurring subscriptions, memberships, and auto-billed services (Gabriel Care reimburses individual wellness treatments and services upon receipt submission. Each claim must be a discrete service or purchase with a dated receipt. Examples: monthly gym memberships, subscription supplement boxes, ongoing auto-ship orders, streaming wellness services)
  • Cosmetic procedures (unless medically indicated)
  • Weight loss surgery
  • Fertility treatments (under review for future inclusion)
  • Services from non-licensed practitioners
  • Claims exceeding monthly wellness balance or tier limits
  • Services received before membership start date

6. Waiting Periods

  • Basic wellness (labs, IV therapy, acupuncture, red light, chiropractic): available immediately
  • Advanced treatments (peptides, psychedelics, stem cells, biological dental, cannabis): 60-day waiting period
  • Pre-existing conditions: 12-month waiting period for related claims, or 6 months with demonstrated health engagement

7. Member Commitment (Catastrophic)

  • Member responsibility depends on the insurance plan and selected Gabriel tier
  • Applies to serious events such as ER visits, hospital stays, and surgery
  • Does NOT apply to wellness claims
  • Gabriel coordinates around the insurance layer and any eligible Gabriel benefit support

8. Insurance Pairing (Optional)

  • Members may pair Gabriel with ACA Marketplace or other compliant insurance coverage
  • Gabriel and its licensed agents may assist with enrollment when authorized
  • ACA plans cover essential health benefits according to carrier and plan terms
  • Gabriel Care supports eligible wellness benefits traditional insurance often excludes
  • Members in mandate states (CA, MA, NJ, DC, RI) should maintain ACA coverage

9. Wearable Health Data

  • Optional but incentivized with lower rates
  • Data used for: health scoring, treatment correlation, personalized recommendations
  • Data encrypted and HIPAA compliant
  • Member can disconnect at any time
  • REMOVED: Wearable discount tiers (Open/Verified/Elite). Wearable tracking is now a FREE feature for all members. Flat pricing: $249/mo Care, $399/mo Care+.

10. Privacy & Data

  • HIPAA compliant
  • Health data encrypted in transit and at rest
  • Never sold to third parties
  • Used only for: member health scoring, claims processing, treatment recommendations
  • Member can request data deletion

11. Community Standards

  • Members agree to use benefits honestly
  • Fraudulent claims result in immediate termination
  • Gabriel uses AI + random audits to verify claims
  • Members agree not to misrepresent services

12. Cancellation & Refunds

  • Cancel anytime with 30 days notice
  • No refund for current billing period
  • Outstanding claims processed normally after cancellation
  • Member commitment obligations survive cancellation

13. Dispute Resolution

  • Contact support first
  • Written dispute reviewed within 14 days
  • Binding arbitration for unresolved disputes
  • Governed by Nevada law

14. Changes to Guidelines

  • Gabriel may update guidelines with 30 days notice
  • Continued membership = acceptance of changes
  • Material changes communicated via email + app notification

  • Members are responsible for their own healthcare decisions
