Medical IV care and wellness IVs are different claims
An infusion used to treat a documented condition, such as certain medicines, iron therapy or medically necessary hydration, can have an insurance pathway. Coverage still depends on benefits, medical necessity, network and authorization.
A cash-pay hydration, vitamin or NAD infusion marketed for general energy, recovery or wellness may not meet the plan's definition of covered medical care. A clinician's recommendation does not by itself guarantee payment.
What to ask the clinic and insurer
Ask before the appointment, because an IV lounge's cash price and a hospital's billed charge can be very different.

It may when hydration is medically necessary for a covered condition and delivered through an approved setting. Elective hydration for general wellness is commonly paid out of pocket.
Does insurance cover NAD IV therapy?
Many plans do not cover NAD infusions marketed for wellness or optimization. Ask the plan about the exact indication, codes and provider before treatment.
Can I use an HSA for an IV?
