This addendum applies when the Gabriel Care enrollment page identifies the Three-Month Start arrangement.
The membership is billed monthly at the ordinary monthly membership fee shown on the enrollment page. Nothing is prepaid. The first monthly fee is charged at enrollment, and each later monthly fee is charged at the start of each monthly billing period.
The membership carries a minimum term of three Monthly Benefit Periods. The member may request cancellation at any time. Cancellation takes effect at the end of the third Monthly Benefit Period or at the end of the then-current billing period, whichever is later. Membership fees continue to be billed monthly through that date, and partial-month refunds are not provided except where required by law.
The membership receives the full tier-specific and household-specific monthly Wellness Pool limit beginning with the first Monthly Benefit Period. The pool resets each Monthly Benefit Period, does not roll over, cannot be aggregated or cashed out, and remains subject to eligibility, documentation, pre-approval, fair-market-value, and reimbursement rules.
The arrangement replaces the Balance Build described in Benefits Schedule A-3 with the full monthly pool in each Monthly Benefit Period. It does not make three months of Wellness Pool value available at once.
Wearable engagement bonuses retain their normal timing and qualification rules. No wearable bonus is available in Periods 1 or 2, and Period 3 eligibility still requires the applicable connection, activity, and other requirements.
After the minimum term, the membership continues month to month at the same monthly fee unless canceled, and ordinary cancellation timing under Section 21 applies.
The person supplying the payment method may be someone other than the covered member. Payment does not add, replace, or change the covered member identified on the enrollment page.
This addendum controls only a conflict concerning the minimum term or the A-3 Balance Build. The Membership and Benefits Agreement and Benefits Schedule otherwise remain in effect.
